What is the Asthma Community Support Program?
The Asthma Community Support Program is a new approach to working with your GP to manage your asthma.
It has been developed by asthma experts and is based on the latest knowledge about treating asthma most effectively.
By seeing your GP about your asthma when you are well and by planning ahead, you are more likely to have fewer symptoms and feel better.
The Asthma Community Support Program has been initiated by the Commonwealth Government with the support of Asthma Foundation Northern Territory.
Who needs an Asthma Community Support Program?
People who have had asthma for a long time as well as people whose asthma has just been diagnosed.
The Asthma Community Support Program is most helpful for people with moderate to severe Asthma.
How will the Asthma Cycle of Care Program help me?
Completing the Asthma Cycle of Care Program with your GP will help you gain control of your asthma and not have your asthma control you.
How does the Cycle of Care Program work?
The Asthma Cycle of Care Program involves 2 visits to your GP for the sole purpose of improving the management of your asthma. During the two visits your GP will:
Fully assess your Asthma;
Review how well your medications are working for you;
Provide you with a written Asthma Action Plan; and
Provide you with information and education on managing your asthma.
Will there be any extra cost for Participating in the Asthma Cycle of Care Program?
The Asthma Cycle of Care Program will involve no extra costs except what it would normally cost to see your GP and buy your medications.
If you or your family experience large health care costs, you may be eligible for assistance through the Commonwealth Government's Pharmaceutical Benefits Scheme (PBS) Safety Net or Medicare Safety Net. Contact details are:
